Please make a separate appointment for each year in which you need a return prepared. Also make a separate appointment for each individual who is having a separate return prepared. Preparing a return for you and a separate return for a parent or child requires a separate appointment for each one.

Documents you will need to bring to your appointment:

• Copy of last year’s federal & state tax return

• W-2 forms showing income from your employer

• 1099-R forms showing pension or annuity income

• SSA-1099 forms showing Social Security benefits

• 1099-DIV,-INT,-MISC,-OID forms showing dividends, interest, NYS tax refund, gambling income, etc

• Health Care information received from the Marketplace, if any

• Any other information concerning your income or deductions

• Photo ID and Social Security/ITIN cards

• Bank information, if direct debit for taxes owed  or direct deposit of refund is desired
